NavigatorACO is a High Needs Population Accountable Care Organization (HNP-ACO) that is part of the ACO Realizing Equity, Access, and Community (REACH) Model introduced by The Centers for Medicare & Medicaid Services (CMS).
Operating under the auspices of Calvary Hospital, NavigatorACO coordinates, communicates, and cares for the changing needs of patients who have two or more chronic conditions, providing an important complementary role within their continuum of care as they approach the end of their life. We provide services in the Bronx, Manhattan, Brooklyn, Queens, Nassau, Westchester, and Rockland counties as well as Connecticut’s New Haven, Middlesex, and New London counties.

Our mission is to provide all-encompassing care to Medicare beneficiaries (patients) who are dealing with complex medical conditions so they are able to access the care they need, when they need it, in the most convenient way. Working with other healthcare professionals and organizations, we simplify the complex clinical, administrative, and logistical issues associated with high-needs patients by providing referrers, patients, and families with coordinated, comprehensive, and compassionate care.
Name and Location
NavigatorACO coordinates, communicates, and cares for the changing needs of patients who have complex care needs. We work with healthcare partners to provide a continuum of care that is based on the patient’s frailty, severity and complexity of illness, and other circumstances.

Voluntarily Align with NavigatorACO
NavigatorACO alignment is voluntary. You can still see your own provider. Even after you have aligned, you are still free to go to any provider, even those outside of NavigatorACO. You are not required to receive care from NavigatorACO. Please note that you may only be assigned to one ACO.
